Four short scenarios. Several bubbles each. No code. ID text box would only accept numbers.No idea how I missed an AC since they were pretty straight forward and even in their own section. Scenarios were weirdly worded and somewhat similar to each other so I guess it's possible I got confused when answering.

I am sorry that your answer did not qualify regarding to attention-check questions. There were 8 attention-check questions in the survey which were informed with an explanation saying 'These questions are to see if you are reading the scenario carefully. Please give the answer based on the scenario above. Based on the answers, you will be qualified to get paid for the survey'.

No code at end. Spot asking for mturk ID does not allow letters, only numbers. I made my own number and emailed the requester. May get rejected but YOLO.
