UPDATE 8/30Turns out I made a mistake by not advancing the screen after receiving my code. Something I've done before but have always managed to catch until now. That's my bad. The requester responded 9 days after I first contacted them and explained in the email they sent me,
"Sorry for the delay in responding!
I manually checked all the approvals and I think I know what happened. Some people forgot to hit "Next" to actually submit the survey in Qualtrics. Therefore, no data was recorded at all and I was unable to approve. Sorry if that happened to you! I went back and tried to make it very clear in the instructions that the "Next" button had to be pressed after I noticed a few people's results were not saved.
I will be launching a couple other surveys like this one in the near future and will pay out the same amount. Keep an eye out for them if you're interested in taking the survey again."
They reversed the rejection and went on to explain the odd feedback I mentioned in the original review,
"Also, just wanted to further apologize because I didn't realize the rejected is sent to Turkers as "x". In the instructions for approvals it doesn't clarify there is supposed to be feedback in this box instead of just marking it as approved or rejected! I'll make sure to provide feedback in the future to rejected HITs."
I in turn apologized for my mistake, thanked them, and am updating the review to reflect how things played out. Whoever it is pulling the strings behind the account on this one seems like good people and I wanted to make sure I owned my part and gave them credit for theirs.

This is one of those fill in all the bubbles about your feelings surveys. Not a terrible one. I completed the survey with no problems whatsoever, received my completion code, and submitted the HIT.

Rejected for unknown reason on a HIT that I encountered zero difficulties completing. No useful feedback given and survey was of a type with no right or wrong answers so I'm reaching out to them for clarification.
I don't know how likely a rejection is but it's certainly a possibility. YMMV
Requester feedback on the rejection is "x"
Seriously. That's what it says under feedback.
"x"
The survey is titled Survey about Well-Being. My own well being. You'd think I would be an authority on the subject. Obviously Noah knows me better than I know myself because 'x'.
Contacting Requester in order to get value for x. Will update in event of a response.
Advice to Requester
The response took much longer than I would have liked but was of good quality when it was received. Requester explained and then did me a solid when they didn't have to.Hedgmog Careful Reader

Pros
Answer generic psych questions about yourself, then receive codeCons
One question per page for at least 70 questionsA lot of the questions seem to repeat theirselves, and are given to you with no sense of continuity
Survey assumes that you took the first part of this before, and has no quals to lock people that didn't take the first survey out
