Rejected claiming I failed an ac which is simply not true, I went over my answers multiple times for just this reason. It took several emails but I eventually got a response that there was an error in the coding and the hit was immediately approved. While it was a fight to get it overturned, the requester was polite and eventually resolved the issue.
