Rejected with the feedback: "Two questions were asked at the end to make sure you had read the document. These questions were simple and you were warned at the beginning of the survey that you would be asked questions. You were wrong on one or both of the questions. Therefore, we cannot consider that you have answered this survey seriously."I don't really remember what was in this study or what the attention check questions could have been. I'm always very careful though and it's very rare I miss easy and fair attention checks. My approval rate is 99.9%. Have emailed the requester will update if they respond.
I actually went back and took a look at the survey again. The two attention check questions are quite unfair if the requester is being this strict about it. The answers are ambiguous for one question and for the other question you need to remember a sentence from pages ago almost word for word as the answers are quite similar and there is even an "I don't know" answer, so it looks like if you are honest and admit you don't have exactly perfect word for word memory of a particular sentence you read previously in the study then you get rejected.
Update 5/7: Requester got back to me a few days ago to let me know they reversed the rejection. HIT still showed as rejected for me though. Waited a day and emailed the requester to let them know it was still rejected. Just noticed today the rejection is now gone.

There is an MC that if you aren't paying very close attention to the exact wording of a phrase you may miss, because they include several variations of the same sentence as options - pretty crappy on req's part
