Edited: Requester responded quickly and said it is now fixed so future people hopefully won't have the issues.Old: Riddled with column/row radio button alignment errors and problems. I took a chance by doing it and emailing the requester with screenshots. Take at your own risk.

You have to continue with out answering on a lot of questions which kind of throws you off. Also, some questions are worded kind of weird, to me at least I had to read them a couple times.
