New requester, no codeUpdate: Rejected with the feedback: "We apologise for rejecting the work but unfortunately we have to make some changes and the ID is essential. Thank you for taking your time to complete the survey."
So it looks like this requester is rejecting because they failed to include a completion code in their HIT. Have emailed them including a screenshot of the final page of the study. Will update if necessary.
Update 4/27: Contacted requester through the email address they gave in the HIT and received a very fast response. Requester claims to be having trouble logging into their mturk account, so they didn't respond through mturk. Apparently they were trying to delete our data (I guess because it was useless without a code) and it ended up rejecting our HITs. Requester apologized and claims to be working on getting our HITs approved.
Update 4/28: Rejection reversed and requester also followed up with me to make sure I was paid.
