Your assumption that the minimum number of days participants work out in a fitness center per week is 1 is objectively idiotic. I haven't been in a "fitness center" in years. The nearest to me is 30 miles away. And there is no circumstance where I would consider joining one, or hiring a personal trainer.

Advice to Requester
You need to take another look at your time estimate. Overestimating that it will take 10 minutes to complete that simple survey, and then offering only 20 cents pay, is GUARANTEED to lose you workers. There was already a return-review about it when I chose to give it a shot, and it barely took a minute (which is acceptable for the pay).
