Advice to Requester
The first set of questions doesn't actually make sense as worded. "Missing work due to smartphone use" is a concept, not a "statement" that can be "agreed" or disagreed with. Either the initial question needs more words, such as "I have had a problem with", or something similar needs to be appended to the individual lines.
