You should only require written justification when someone deems a video/title/cover/etc. to be offensive or inappropriate. Additionally, I was unsure what you mean to be "inappropriate" or "offensive". Some people are very sensitive to things that would be acceptable in G-rated movies! Some clarification would be nice.
