- No code, just kind of stops randomly may or may not be an error that gets the HIT rejectedUpdate: Hit was rejected, most likely because of the lack of code. I contacted the requester ask for a reversal and let them know the next step would be contacting the University of Nevada, Reno IRB and submitting a complaint
Update: Got the first response back from the requester after sending a second email. They uploaded the wrong survey and just rejected everyone, responded back with that's still not okay next step is IRB without reversal.

Didn't notice the rejections and low approval until I got to the end and saw there was no code, I would have just returned it if I had noticed earlier.
